If you or your loved one has been diagnosed with congestive heart failure (CHF), ongoing treatment and monitoring is crucial. Our highly-trained team provides a comprehensive assessment and along with their physician develops an individual treatment plan that addresses their unique needs. Physical and Occupational therapies can be added for activity tolerance and endurance training. Patients can be referred by their outpatient physician or when being discharged from the inpatient setting.
